Center For Primary Care office and privacy policies are posted in our waiting rooms and a copy of each is provided to all new patients when they check in. You may request a copy at any time.
Office Policies
Appointments
As a courtesy, we will call to remind you of your next scheduled visit. If you are unable to keep your appointment, please call our office at least 24 hours before your appointment to cancel or reschedule so we can give your appointment to another patient.
Insurance
CPC accepts virtually all health insurance plans available in the CSRA. Please bring your insurance card with you each time you visit. If you have questions regarding acceptance of your insurance, give us a call or consult with your carrier.
Payment
Please be prepared to pay your deductible, co-pay, or office fee at the time of service. CPC accepts Mastercard, Visa, cash and checks.
Privacy Policy
When visiting CPC, you either have been or will be given a copy of our privacy policy and asked to sign a form acknowledging that you have received it and that you authorize CPC to use and disclose your protected health information as described in the privacy policy. The reason we are asking you to do this is to ensure that you have been informed about HIPAA, the Health Insurance Portability and Accountability Act, and how it is implemented at CPC. It ensures you are aware of your rights as a patient, and that you are aware of the procedure for making requests or filing complaints.
The privacy portion of the federal law known as HIPAA is designed to protect your privacy and the confidentiality of your medical information. It applies to CPC as your healthcare provider along with other agencies involved in your healthcare. The provisions of this law has not changed the way we interact with you or handle your records at the Center for Primary Care because it has always been our policy to protect your privacy and to guard access to your personal health information to as great a degree as possible.
One of our priorities in implementing HIPAA is to do so in such a way that it is easy to understand, does not interfere with the quality of care we are able to provide to you, and is not inconvenient for you. Additionally, our staff is available to address whatever questions or concerns you might have about these policies and to assist you in any way possible.
In providing your healthcare, CPC protects your privacy as a patient of this practice and keeps your medical information confidential to as great an extent as possible. However, if you have any questions or concerns or if you want to take any action related to your medical records maintained at CPC, we ask that you notify our privacy officer (Melody Collins, 706-922-8242)
